CDCFIB Requirements for Physical Screening and Document Verification 

Once You are shortlisted for the CDCFIB after the CBT stage, here are the list of the documents you need to provide during the physical screening and documents verification.

All applicants invited for physical screening and document verification must attend with the ORIGINAL

documents listed below. Where applicable, please also bring clear photocopies.

REQUIRED DOCUMENT

• Recent passport photograph
• Educational certificates
• Birth certificate /Age declaration
• Letter of indigeneity
• Medical fitness report
• National ldentity Number (NIN)
• Bank Verification Number (BVN)
• Acknowledgement slip
• Letter of good conduct
• Guarantor's form
• Examination slip

DETAILS ACCEPTABLE EXAMPLES

• Passport-size photo with a plain white background
• SSCE, ND, NCE, HND, Bachelor's degree, etc.
• Birth certificate or declaration of age issued by NPC,
• Court, or recognized government hospital
• State of origin certificate
• Issued by a government hospital
• Provide NIN slip or valid NIN documentation
• Provide BVN slip or bank confirmation
• Receipt/acknowledgement from the recruitment portal
• Issued by the area head or district authority
• Completed guarantor information form (as required by CDCFIB)
• Original examination/test slip provided during registration

IMPORTANT: Ensure all original documents are valid and legible. Bring photocopies of each document for submission.

Failure to present the required documents may disqualifty you from further screening.
